Nukes in really leave little radiation. The principal sources of contamination would be the nuclear power plants which weren't numerous. If the cities are evacuated, the human losses would be very probably lower than the loss of 1/2 of population during the fall of USSR.

You apparently know nothing about nuclear weapons. Tell that to the Marshall Islanders that survived the Castle test.

You can have many exemples which lead to a different conclusion. Tsar bomba had a little radiactive contamination.

SIOP-62, the US nuclear war plan at the time of the Kennedy Administration was, if activated, expected to kill upwards of 108,000,000 Soviet citizens and wipe out 80-85 percent of the USSR's industrial capacity in one blow. The land would be poisoned and any meaningful military and civilian infrastructure shattered. The country would have essentially ceased to exist.

Fast forward to 1990, and even one giant US nuclear submarine could carry 192 warheads, capable of killing or maiming upwards of 50 million people. Even today the US still has 18 such submarines in its arsenal.
