    I'm a big fan of the Rare Earth series, and this episode was an eyeopener to watch.

    The same underlying ideas are why Fukushima happened in large part. The management of the plant said it was 100% safe. Therefore, it had to be 100% safe-- as it was. This left no room for improvement. Thus, when disaster hit, and the plant got creamed, it was for management to fall on the proverbial sword while the plant struggled to contain the damage and mitigate it.

      I wish our politicians and big business types would fall on their swords when they screw up.
